Brain part aside (the transplant didn't work!), you're right. It would have taken the Initial Broadside bonus to equal zero, but unless his HDT was set higher, it should have fired. And if that were the case, his HDT would have been indicated.

I'm showing the above turn is 16. Has it progressed since, and what would the notes indicate? Turn 15 indicates her having fired, which should have removed his IB bonus for turn 16.

Seems like there's a hidden bug in the firing routine that pops up now and then; the manifestations are usually similar. I've found it very important to make certain that the proper broadside is checked, just to make sure. The AI sure AIn't too bright sometimes.
